School's Clinic/ Ryan Health

Ryan center is a School-Based Health Clinic at Park West Campus. The school clinic is currently open and available to see students who attend HSHM. They can see both in-person and remote students. Below you will find a  Spanish and English fillable PDF consent forms that students should complete and have their parent or guardian sign. Once completed, they can email the form to: school@ryanhealth.org OR they can bring the forms to the school-clinic.

Students are required to have these forms completed prior to their first visit. If a student has already completed the consent form in the past, they do not have to complete a new one. Please note that if a student is older than 18 years of age, they can come to the clinic at any time and complete the adult consent form.
